Of course there is paperwork! In your acceptance pack there are various form that you MUST complete before you leave for overseas. Please go through your acceptance pack carefully and follow the instructions you have received from your program adviser. Keep the following points in mind:
- Some forms will have exact due dates. If they do, make sure you follow them. If there are no exact dates given for turning in your paperwork, make sure Albany has everything at least one month before the start of your program.
- Some forms require a parent's signature if you are under 21. Your birthday may be coming, but student signatures are valid only for those participants over 21 at the time of signing.
- Some forms must be signed in the presence of a notary. The notary witnesses the signature and certifies that you have signed it. There are notaries on the Albany campus in the Office of International Education and in CC 358, and most banks have a notary in their offices.
- Albany students participating on programs run by other SUNYs will receive most of their required forms from the sponsoring SUNY campus but need to come to the Office of International Education at Albany for all paperwork related to registration and course approvals.
FORMS
IMPORTANT: Completion and submission of these forms is a condition of participation in programs sponsored by the University at Albany. The Office of International Education at the University at Albany reserves the right to cancel a student's participation in a program if any form indicates a substantial reason to do so, or if all of the required forms are not on file at least ten days prior to the beginning of the program. It is the responsibility of each participant to ascertain that all paperwork is in order.
Participants who have not signed and returned these forms have implicitly accepted all conditions and regulations. Please refer to the adviser's checklist for your program and keep track of when you submit each form.
